Who Fills This Form?

This form must be filled by the Authorised Representative (PS / PA / CC) of the dignitary. The representative registers using their own details.

The dignitary's details are entered in the Application step.

Note: The dignitary's name, title and designation are entered in the Application step — not here.
The mobile number you register with is your permanent login ID and will be used to verify your identity if your PIN is lost.
